This prospective cohort study suggests that there are few differences in the 2 year clinical course between ACL-injured patients who choose nonsurgical and surgical treatment. While surgically treated patients had a significantly higher crude risk of knee reinjury, there was no significant difference in the risk after adjusting for age and preinjury sports participation. Patients in both groups showed large improvements in knee function; however, at 2 years, one fifth of patients reported knee reinjuries and a considerable number of patients still exhibited functional deficits. Future work on how to identify and better treat the patients who will have an unsatisfactory outcome is needed.no_NO
